The YYGP sets out what you, as a patient, can expect from your GP practice, alongside how you can help us to provide the best care for you and our wider community.
In line with the guidance, our practice has also nominated a YYGP Lead, our Practice Manager, who will act as a point of contact should you wish to provide feedback or raise any concerns about the charter.
