We are registering new patients.

You may apply to register with us if you live within our catchment area.

Requirements

Proof of Photo Identity

  • Passport,
  • Full UK Driving Licence,
  • BRP or
  • UK Home Office Letter

Proof of address

  • Utility Bill (Gas, Electricity, Water or Telephone),
  • Council Tax,
  • Full UK Driving Licence,
  • Bank or Building Society Statement,
  • House or Motor Insurance Certificate,
  • Tenancy Agreement,
  • Pension Notification Letter,
  • HMRC Tax Notification Letter,
  • University or College Letter or
  • UK Home Office Letter

Registration Process

Step 1

  • Complete Registration Form & Questionnaire

Step 2

  • Send an email to info.e12medicalcentre@nhs.net
    • Subject: New Patient Registration
    • Attach proof of Photo Identity & address with your name, date of birth & contact number.

Please make sure, you provide correct:

  • Mobile number: Practice to arrange a new patient health check appointment
  • Email address: Practice to email you your online Patient Access (How to register with Patient Access (PDF, 2.03MB))

Using Patient Access (after completion of registration), you can:

  • Book appointment
  • Order Repeat Medications
  • See your Medical Records

If you don’t receive a call from the practice to arrange a New Patient Registration appointment within 72 hours of sending your email, please contact the GP practice.

Our practice policy has always been to have health check first at the time of registration either with our nurse or with our healthcare assistant. This enables us to update your records and clinicians to have better understanding of your health status.

New Patient Health Check: Mandatory to complete your GP registration.

Following checks may be required during the check

  • Chlamydia Test (aged 16 to 24 years) - Download a Leaflet (PDF, 257KB)
  • HIV Test - All new patients
  • TB (Tuberculosis) Test (aged 16 to 35 years and living in the UK for less than 5 years)
  • NHS Health Check (40 to 74 years and No Chronic disease)
